Our hybrid model combines in-person and virtual care to meet patients wherever they are. Weāre rapidly expanding and building a national virtual care team to support patients across all 50 states.
On-Site Requirements: Remote (U.S. Based)
Schedule: Full-Time, Monday-Friday
Weāre looking for an experienced Virtual Care Coordinator to join our growing team. In this role, youāll support patients and clinicians across the country by managing virtual patient check-ins, handling inbox messages, and ensuring seamless care coordination in a fully remote environment. This is an ideal opportunity for someone with prior virtual care experience who is comfortable navigating EMRs and multitasking in a fast-paced, digital-first clinic in a health tech startup environment.
This role reports to the Nationwide Virtual Care Practice Manager.
Responsibilities::
⢠Conduct remote patient check-ins and pre-visit preparation including medication reconciliation, health history intake, and screening questionnaires
⢠Document medical information accurately in the EMR and prep charts ahead of clinician visits
⢠Communicate with patients regarding upcoming appointments, test results, and follow-up instructions
⢠Support clinicians during virtual visits by entering real-time documentation, assisting with orders, and coordinating next steps
⢠Coordinate care: manage referrals, prior authorizations, and medication refill requests
⢠Ensure prompt and professional communication with patients, clinicians, and care team members
⢠Collaborate with the virtual care team to identify and improve workflows
⢠Maintain a warm, inclusive, and professional patient experience aligned with knownwellās values
Requirements: :
⢠High school diploma or equivalent
⢠2+ years of experience in a virtual care, patient coordination, or healthcare support role
⢠Familiarity with virtual care workflows in a primary care or chronic care setting
⢠Prior experience supporting care in a multi-state, remote care delivery model
⢠Strong communication and organizational skills
⢠Comfort with remote tools, secure messaging, EMR platforms (Athena preferred)
⢠Tech-savvy, adaptable, and comfortable operating independently in a remote setting
⢠Knowledge of obesity medicine, weight management, or chronic care workflows (preferred)
⢠Familiarity with HIPAA-compliant telehealth platforms (preferred)
⢠Experience with Athena EHR (preferred)
