Over Ambulant Centrum Utrecht

In het Ambulant Centrum Utrecht zijn de poliklinieken voor jeugd en volwassenen van Fivoor gevestigd. Het ambulante behandelaanbod richt zich op jongeren en volwassenen die norm-overschrijdend gedrag vertonen in combinatie met psychiatrische problematiek. Er is altijd sprake van complexe problematiek en geregeld ook uitval op verschillende levensgebieden, zoals onderwijs, werk, wonen en sociale relaties. Er zijn van Fivoor 2 forensische fact teams en 1 poli team actief binnen de regio Utrecht.

Wat ga je doen?

Diagnostiek en behandeling van volwassen patiënten die normoverschrijdend gedrag vertonen, vaak in combinatie met een psychiatrische, persoonlijkheid en/of ontwikkelingsstoornis, is je dagelijkse werk. Je krijgt als verpleegkundig specialist de ruimte om je eigen rol en deskundigheid vorm te geven binnen de bestaande structuur. Behandeling van delict- of risicogedrag staat weliswaar centraal, maar er is uiteraard ook aandacht voor behandeling van de overige problematiek. Patiënten komen zowel in een vrijwillig als gedwongen kader in behandeling, (eigen) motivatie is een belangrijke factor in een geslaagde behandeling.

Het gaat om een uitdagende en complexe doelgroep waarvoor passende zorg in samenwerking met ketenpartners aangeboden wordt. In de functie krijg je naast je contact met cliënten ook veelvuldig te maken met verwijzers zoals reclassering, ggz en maatschappelijke instellingen.Als verpleegkundig specialist ben je regiebehandelaar en werk je samen in een multidisciplinair team om de patiënten de best passende zorg te bieden. In overleg wordt gekeken welke patiënt bij wiens expertise aansluit. Uitvoeren van complexe diagnostiek is een onderdeel van de werkzaamheden, net als meedenken over en vormgeven van beleid bij crisis (zowel gericht op delictgedrag, suïcidaliteit of zorg om kinderen/meldcode huiselijk geweld). Er wordt gewerkt volgens het RNR (Risk-Need-Responsivity) model, waarbij er een risicotaxatie en delictanalyse wordt afgenomen. Medicamenteuze behandeling (en motivatie hiertoe vergroten) is geregeld een onderdeel van het behandelaanbod. De voornaamste behandelmethodes zijn psycho-educatie, CGT (al dan niet op delictgedrag), schematherapie en verschillende vormen van traumabehandeling, waaronder EMDR.

Op het ambulante centrum in Utrecht werk je samen met verschillende disciplines. Binnen het ambulant centrum bieden we ook een opleidingsplaats voor de Verpleegkundige in opleiding tot Specialist.
