VCOH Service Coordinator

Vermont Comforts of Home (VCOH) offers adults a choice to remain in the community when they are no longer able to live on their own and also require a nursing home level of care.

VCOH is a person-centered program.

Service Coordinators meet at least monthly with the shared living provider and participant in their home to support and manage various aspects of the state-required expectations to ensure a healthy environment that meets the participant’s goals and needs.

Mix of field work, work from home and main office in Essex Jct. at least two days per week.

Field work located in the following counties:  Franklin County

Background checks are required

Must have valid driver’s license and insured vehicle

Generous benefits, holiday schedule and compensation package

Thorough initial and ongoing training and support for Service Coordinators

Mileage reimbursement provided for field work visits

M-F 8am-4pm and occasional evening or weekend availability

College degree in Human Services or related field preferred

We are offering a generous sign on bonus!! $500 after successfully completing six months and another $500 upon successfully completing one year at Upper Valley Services.
