Overview
We are seeking a compassionate and clinically strong Traveling Consultant Pharmacist to join our team in supporting a PACE (Program of All-Inclusive Care for the Elderly) program in Fort Gratiot, MI. This role is ideal for a pharmacist passionate about geriatric care and interdisciplinary, patient-centered practice.
As a key member of the care team, you will help optimize medication therapy and improve health outcomes for older adults with complex medical needs.
Key Responsibilities
Review and manage medication regimens for PACE participants
Conduct medication reconcilliations and detailed chart reviews
Identify, resolve, and prevent medication-related problems
Particpate in interdisciplinary team (IDT) meetings and care planning
Ensure compliance with CMS regulations, state pharmacy laws, and PACE standards
Provide education to participants, caregivers, and healthcare team members
Qualifications
Doctor of Pharmacy (PharmD) from an accredited program
Active Pharmacist License (required)
Active Controlled Substance License (required)
2+ years of experience in geriatrics, long-term care (LTC), or related setting
PACE experience preferred
Willingness to travel
