Under the direction of the Manager and in collaboration with Homeless and Housing Help Hub (H4) Staff within a multidisciplinary team, the Mental Health Support Worker will provide support to the H4 clients. The incumbent is responsible for assertive outreach to individuals within the Shelters. The position will work with individuals to gain rapport, educate, and establish community linkages and treatment plans.
The Support Worker will provide a continuum of functions, including but not limited to: assertive outreach, building therapeutic relationships, developing plans with clients, client and systems advocacy, symptom management, life skills teaching, supportive counselling, and crisis intervention.
RESPONSIBILITIES & DUTIES:
In collaboration with the Homeless and Housing Help Hub Staff, the Mental Health Support Worker will be responsible for:
Assertive outreach to individuals in Shelters.
Establishing rapport and forming trusting relationships that provide the opportunity to assess needs and gain mutual agreement with the client on a treatment plan.
Working with a multi-agency team at point of care, improving case planning by assessing needs and gaining agreement on a plan of care.
Improving access to care by conducting case planning, promoting linkages to appropriate resources, and by providing education and assistance interfacing with the health care and social service systems.
Monitoring clients while in the Shelter care to ensure safety.
Link clients with other housing options.
Life Skills – Activities of Daily Living (in collaboration with H4 Staff)
Facilitating and supporting the achievement of the components of the care plan specific to addressing activities of daily living.
Providing skill-development on an individual or small group basis to support the development of life skills to maintain their home (such as budgeting, cooking, cleaning, hygiene, etc.).
Providing supportive intervention and problem-solving with clients specific to practical issues related to activities of daily living.
Providing ongoing support through positive and constructive feedback, encouragement and informal evaluation.
Providing life skills support to individuals in the housing program to support residential tenancy skills.
Community Development
Keeping abreast of other resources in the community;
Maintaining professional contact with commonly accessed organizations so that purposeful interaction on behalf of clients can be effectively accomplished; and
Reducing stigma by actively working towards overcoming barriers to community integration.
Program Evaluation and Development
Identifying gaps, interruptions or disruptions in services;
Assessing client program satisfaction;
Determining efficacy of formal services and informal support; and
Actively participating in improving the quality of programs and service delivery.
Ensures objectives of program are met.
Participating in the evaluation of program goals and objectives making recommendations on changes to current program activities for improved service delivery.
Working toward the best interest of the client as a member of the CMHA team.
Ensures a minimum workload of direct client intervention as negotiated with the immediate supervisor
Ensures all file management and reporting functions are maintained in a comprehensive and timely manner.
Ensuring all client contact is recorded appropriately.
Ensuring that monthly, annual, and other required reporting documentation are completed.
Maintains general knowledge of the Mental Health Act and related legislation.
Represents program and agency in a professional manner.
Substitutes for, and assists other agency staff where skills are applicable.
Providing additional support to unit staff as required.
EDUCATION & SKILLS:
Knowledge and skill acquired through the completion of Bachelor of Social Work degree (BSW) from an accredited University is required. Registered member of the Ontario College of Social Workers and Social Service Workers and in good standing.
Comprehensive knowledge of mental illnesses, signs and symptoms, as well as planning and organizational skills. Strong assessment skills. Knowledge of community resources
One year recent and relevant experience, in the judgement of the employer, working with and teaching life skills to vulnerable individuals, particularly those with a mental illness or related diagnosis.
Ability to communicate effectively with clients, public, professionals and staff both in written and oral form. Ability to write clearly and to effectively present ideas and document activities; demonstrates the ability to read and interpret written information. Demonstrated ability to communicate with others in a respectful and helpful manner while simultaneously building credibility and rapport. Demonstrates good organizational skills required for tracking multiple issues/tasks in a wide array of situations. Skilled in developing materials and facilitating groups.
Computer literacy skills and working knowledge or various Microsoft Windows application is required.
Bilingualism in both official languages at the advanced level is considered an asset.
A valid Ontario Driver’s License with access to a reliable vehicle and proof of current insurance and licensing are required for this position.
Hours of Work: Thirty-Five (35) hour work week. The regular hours of operation are Monday to Friday, 10:00AM to 6:00PM. Flexible hours are required to meet client and service needs.
Salary range: Grade 9, $37.84 – $44.81 per hour
Per 2022 - 2026 Collective Agreement
Applicable shift premiums are paid in accordance with the Collective Agreement, Article 17.11.
