TEMPORARY UNTIL DECEMBER 31 2026

Under the direction of the Clinical Operations Manager and working in close liaison with the Community Partner Leadership team, this position supports clients through the delivery of comprehensive mental health assessments and supportive counselling and the navigation of mental health and addiction services provided by the agency and in the community.

 

These services may change subject to on-going needs assessment of services required by the Community Partner.

 

RESPONSIBILITIES & DUTIES:

Provide mental health/addition supports as required and based on program needs.

Conduct and complete mental health and addictions needs assessments of referred individuals seeking services to a mental health outpatient program from CMHA or where deemed appropriate to an external service provider.

Screen clients to determine most appropriate mental health and/or addiction services.

Provides supportive counselling until entry into an appropriate program within CMHA or other recommended service providers.

With input from the client, family (as requested by the client) and the care team, create outcome goals, and an overall care plan to ensure seamless implementation of services recommendation(s) and transfer of care where appropriate.

Implement survey tools as needed in collaboration with management to determine most appropriate mental health/addiction supports.

Collaborate where appropriate with Community Agencies to help navigate client’s through community resources in developing a care plan and obtaining goals

Attend care planning meetings and work with the other community providers on cohesive treatment plans

Contact the identified community resource and or referral source to ensure transfer of care is seamless.

Provide comprehensive information to client on various community resources as it relates to their care plan goals.

Coordinate involvement of other agency staff and outside resources.

Provide support as needed during the transition of care process.

Liaise with family/significant others and natural supports where appropriate, to facility the client’s links within the community.

Develop goal focused psycho-education sessions based on client needs.

EDUCATION & SKILLS:

Requires knowledge and skills acquired through the completion of a University degree as deemed relevant by the employer (i.e BSW, BScN.).

Registered member in good standing of a professional college deemed relevant by the employer such as the College of Nurses (preferably with Canadian Certification in Psychiatric/Mental Health Nursing) or the College of Social Workers in good standing is required.

Two (2) years recent and relevant mental health clinical work and supportive counselling experience, in the judgment of the employer, with vulnerable individuals with serious mental illness and addictions. Experience working with the unhoused population is an asset.

Comprehensive knowledge of mental illness and treatments;

Comprehensive knowledge of addictions services, both inpatient and outpatient programs, and completing appropriate assessments and referrals.

Up to date knowledge of relevant mental health policy and legislation including the Mental Health Act;

Knowledge of community resources;

Demonstrates the ability to work in a multidisciplinary team

Computer literacy skills and working knowledge or various Microsoft Windows application is required.

Bilingualism in both official languages at the advanced level is considered an asset.

A valid Ontario Driver’s License with access to a reliable vehicle and proof of current insurance and licensing are required for this position.

 

 

Hours of Work:       Thirty-Five (35) hour work week. The agency’s regular hours of operation are Monday to Friday, 8:30 AM to 4:30 PM. Flexible hours are required to meet client and service needs.

 

Salary range:          Grade 9, $37.84 – $44.81 per hour

                                 Per 2022 - 2026 Collective Agreement

Applicable shift premiums are paid in accordance with the Collective Agreement, Article 17.11.
