Team Lead- Patient Care Coordination
About the Role
We’re looking for a motivated Team Lead to manage a team of Patient Care Coordinators (PCCs). In this role, you’ll help recruit your team, coach and mentor your team, oversee day-to-day operations, and ensure quality care for patients. You’ll also play a critical role in maintaining compliance with HIPAA and company standards, while helping shape a culture of accountability and growth.
What You’ll Do
Lead, mentor, and support a team of PCCs to achieve daily performance goals.
Monitor productivity, quality, and compliance metrics.
Ensure all patient care activities meet HIPAA and confidentiality requirements.
Provide coaching, feedback, and career development opportunities.
Collaborate with referral partners and internal teams to ensure smooth patient care operations.
Drive innovation and initiatives to improve our patient experience.
What We’re Looking For
Experience in healthcare administration, patient coordination, or a related field.
Leadership experience (formal or informal).
Knowledge of HIPAA and compliance standards.
Strong communication, organizational, and problem-solving skills.
Individuals who are curious, creative and driven to drive change.
Bachelor’s degree in a related field preferred (or equivalent experience).
