Position Overview
We are seeking compassionate and motivated Case Managers to join our team. This role involves conducting patient evaluations through in-office consultations and video teleconferencing, as well as coordinating care and connecting individuals to essential services.
Key Responsibilities
Conduct patient assessments via video teleconference and in-office consultations
Identify patient needs and coordinate access to medical, social, educational, and community resources
Collaborate with families, providers, and community organizations to support individualized care plans
Maintain accurate patient records, including social and medical histories
Prepare and submit assessments, progress notes, and reports using the electronic health records (EHR) system
Provide consultations via telephone if teleconferencing is unavailable
Ensure adherence to company policies and procedures, prioritizing patient safety
Required Qualifications
Bachelor’s degree in a relevant field from an accredited college or university OR CADC/CADAC certification
Ability to obtain Targeted Case Management certification within six (6) months of employment
Professional liability insurance with minimum coverage of $1,000,000 per occurrence / $3,000,000 aggregate
Commitment to completing ongoing continuing education
Preferred Experience
At least one (1) year of experience in case management or behavioral health
