The Targeted Case Manager/Group Care Assistant is the lead role in coordination of care between the child and outside entities, providing leadership and advocacy in building effective relationships between treatment teams, families and external stakeholders. The position is the principle participant in the process of referrals, assessments, intakes, admissions, treatment planning, family involvement, ongoing authorization and discharge.

Ensure timely and accurate documentation of services, reports and records
Complete billing for case management services within required timeframe

What You Need to Get the Job Done:
Applicants must meet our state licensing requirement minimum age of 21 at the time of hire along with these basic qualifications:
A bachelor's degree in a human services field, preferably in social work
One year of full time experience working with children
Knowledge of community resources and social work practices with at risk children
Have an understanding of the child protective, youth court and mental health systems in Montana
Valid driver's license
Ability to be an outgoing team player who can work collaboratively and personably with other staff
Ability to lift 25 lbs as needed
Ability to climb stairs as needed
