EPIC Health Partners is expanding its Substance Use Case Management team and is seeking dedicated, compassionate Substance Use Case Managers to support individuals and families impacted by substance use and co‑occurring disorders. As our program continues to grow, this role is essential in ensuring individuals receive coordinated, person‑centered, and culturally responsive services that promote recovery, stability, and community integration.
This position plays a vital role in care coordination, advocacy, and collaboration across interdisciplinary teams to support positive outcomes for those we serve.
Essential Duties & Responsibilities
Assess individual needs and develop substance use case management Individual Service Plans (ISPs)
Enhance community integration opportunities and connect individuals to appropriate community supports
Assist individuals in locating, developing, or obtaining needed services, resources, public benefits, and transportation
Ensure coordination of services within EPIC Health Partners, with external providers, and with human service systems including local health and social services agencies
Monitor service delivery through regular contact with individuals, service providers, and through site or home visits to assess quality of care
Advocate for individuals in response to changing needs and update ISPs accordingly
Lead and participate in interdisciplinary team meetings and facilitate communication regarding attendance, engagement, and treatment progress
Assess the need for changes or additions to services, facilitate referrals, and ensure ISPs remain current
Attend required program activities
Complete all required documentation accurately and timely
Maintain confidentiality and security of Protected Health Information
Qualifications
Bachelor’s degree in psychology, social work, counseling, vocational rehabilitation, or another VA DMAS‑approved Human Services field required
At least one year of experience providing substance use or mental health services
Staff must be currently certified in: CPR, First Aid, and have verification of Tuberculosis screening.
Familiarity with substance use treatment and trauma‑informed care principles
Knowledge of DBHDS/DMAS standards and documentation practices
Ability to successfully complete required background and registry checks
