Position Summary
The Street Outreach Supervisor leads field-based outreach operations with a dual focus on generating high-quality member referrals into CalAIM Enhanced Care Management (ECM) and Community Supports (CS) and Behavioral Health (BH) and building durable, strategic relationships with community-based organizations (CBOs) across multiple counties and health plans.
This role is accountable for outreach performance, referral pipeline development, and the establishment of trusted partnerships that drive sustained referral flow, coordinated care, and improved member outcomes.
Salary - $80,000 - $90,000 annually
Key Responsibilities
Referral Generation & Pipeline Development
• Lead outreach strategies designed to consistently identify, engage, and refer ECM- and CS-eligible Medi-Cal members
• Establish clear expectations for outreach staff related to referral targets, productivity, and conversion outcomes
• Monitor referral volume, referral quality, and enrollment conversion rates across counties and health plans
• Identify barriers to referral acceptance and implement corrective strategies
Community-Based Organization (CBO) Partnerships (Strategic Focus)
• Establish, cultivate, and sustain long-term relationships with community-based organizations, including shelters, housing providers, street medicine teams, behavioral health providers, substance use treatment programs, and faith- or culturally based organizations
• Develop strategic referral partnerships that support ECM and CS enrollment and service delivery
• Position the outreach team as a trusted, reliable referral partner within each county’s service ecosystem
• Participate in community coalitions, case conferencing, and coordinated entry or outreach meetings
• Formalize referral workflows and communication pathways with key CBO partners when appropriate
Staff Supervision & Performance Management
• Supervise and coach outreach staff with an emphasis on relationship-building, referral generation, and documentation quality
• Train staff on how to engage CBO partners effectively and professionally
• Conduct regular performance reviews tied to outreach productivity and partnership development
Outreach Operations
• Oversee daily outreach activities across multiple counties, prioritizing locations and partners that yield sustainable referral opportunities
• Support staff in complex field-based engagement, enrollment, and partner coordination efforts
• Ensure outreach coverage aligns with ECM and CS growth and access priorities
ECM & Community Supports Program Integration
• Maintain strong working knowledge of ECM eligibility pathways and available Community Supports
• Coordinate closely with ECM Care Managers and CS operational teams to ensure warm handoffs and timely intake
• Translate health plan and county priorities into targeted outreach and partnership strategies
Cross-County & Health Plan Coordination
• Serve as a key point of contact for outreach-related referral activity across multiple counties and health plans
• Participate in performance review meetings related to referrals, partnerships, and access
• Support audits and reporting related to outreach, referrals, and partner engagement
Documentation, Data & Quality Improvement
• Ensure accurate and timely documentation supporting referrals and partner interactions
• Use data to assess referral trends, partner performance, and outreach effectiveness
• Support continuous quality improvement initiatives tied to referral generation and community partnerships
