Sr Backend Engineer (Node.js)

We are building a new, large-scale multi-tenant healthcare platform designed for therapists to manage and document session notes securely and efficiently.

The product is currently in development (not a migration), with Figma designs already completed and a Flutter-based frontend in place. The system is being architected for scalability and long-term growth, with plans to expand the backend team after the first successful hire.

This is a high-impact opportunity to shape the backend architecture of a healthcare application from an early stage while collaborating directly with technical and business stakeholders.

About the Role

We are looking for a Senior Backend Developer to partner closely with the frontend lead (Flutter).

You will be the primary backend expert, working on a multi-tenant architecture and contributing to key decisions around scalability, performance, and infrastructure.

This role requires strong autonomy, clear communication skills, and comfort interacting directly with internal stakeholders such as supervisors and coordinators.

The development approach is design-driven, prioritizing product clarity and execution over strict adherence to Agile or Waterfall frameworks.

Key Responsibilities

• Design and implement a scalable multi-tenant backend architecture
• Develop and maintain APIs using Node.js and Nest.js
• Work with Prisma as the ORM layer
• Architect and manage cloud infrastructure in AWS
• Set up and maintain CI/CD pipelines
• Ensure best practices in security, performance, and maintainability
• Collaborate closely with the frontend developer (Flutter)
• Contribute to architectural decisions and backend standards
• Communicate directly with internal stakeholders to clarify requirements and ensure alignment

Requirements:
• Strong experience with Node.js + Nest.js
• Experience working with Prisma
• Proven experience with AWS (architecture-level understanding preferred)
• Experience implementing and maintaining CI/CD pipelines
• Ability to work independently with high ownership
• Experience in the healthcare, education, or medical industry
• Fluent English (written and spoken)
• Experience contributing to architectural decisions in complex systems

• Experience working alongside Flutter teams
• Experience designing multi-tenant systems from early-stage architecture
