Als Sociaal Psychiatrisch Verpleegkundige (SPV) bij Poli Autismespectrumstoornis(ASS)/ aandachtstekort-hyperactiviteitstoornis (ADHD) speel je een cruciale rol in het bieden van coördinerend regiebehandelaarschap en specialistische zorg aan cliënten met ASS of ADHD. Samen met een multidisciplinair team werk je aan een integrale benadering van zorg, waarbij je de gezondheidstoestand beoordeelt, behandelingen uitvoert, behandelplannen opstelt en interventies coördineert. Je biedt psycho-educatie en therapieën aan om cliënten te ondersteunen bij het omgaan met de uitdagingen van ASS/ADHD.

Als Sociaal Psychiatrisch Verpleegkundige (SPV) bij Poli autisme/ADHD ben je verantwoordelijk voor het bieden van gespecialiseerde zorg en ondersteuning aan cliënten met ASS/ADHD. Je speelt een essentiële rol als regiebehandelaar in het behandelteam en werkt nauw samen met andere professionals om een integrale benadering van zorg te bieden.

Je beoordeelt en monitort de psychische, sociale en lichamelijke gezondheidstoestand van de cliënten en voert behandeling uit. Je registreert en rapporteert de voortgang van de behandeling en evalueert de effectiviteit van de interventies. In samenwerking met het multidisciplinaire team en de cliënt ben je verantwoordelijk voor het opstellen en bijstellen van de behandelplannen. Hierbij heb je aandacht voor de specifieke behoeften en doelen van de cliënt. Ook verleen je psycho-educatie. Je biedt individuele- en groepstherapieën aan en ondersteunt op die manier cliënten bij het omgaan met de uitdagingen van ASS/ADHD.

Je coördineert en regisseert het zorgproces en bewaakt hierbij de voortgang en zet, indien nodig, de juiste interventies in. Je werkt nauw samen met verschillende disciplines, waaronder psychiater, verpleegkundig specialist, psychologen, GZ psychologen, medisch secretariaat en collega SPV’ers om een geïntegreerde aanpak van zorg te waarborgen.
