Job Description

Role of a Sponsored Residential Provider

Sponsored residential service is a DBHDS licensed service that provides individuals with
developmental disabilities the opportunity to be sponsored by an individual or family providing
services in their home. The Sponsor adheres to a person-centered plan of support to help
individuals strengthen life skills, achieve their personal goals, and build independence. The
Sponsor aids individuals with skill-building, as well as daily routine supports, general supports,
and safety supports, to include medication management, socialization, and adaptive skills.

Duties and Responsibilities:

● Support individuals with daily living needs, to include hygiene and personal care as
needed
● Provide skill-building training and support
● Communicate and understand individuals receiving services
Provide the care, treatment, training, or habilitation for individuals receiving services in
the home
● All members of the sponsored household to accept individuals with disabilities and their
disability-related characteristics, especially the ability of children in the household to
adjust to nonfamily members living with them
● Teach and demonstrate advocacy skills and communicate with Program Director,
Guardians, Payees, and other professionals on behalf of the individual
● Provide monitoring and supervision of individual for health and safety
● Provide daily meals in the home and/or community setting to provide for nutritional
needs of individual(s)
● Provide support with medication management and medical appointments & concerns
● Maintain the physical condition of the Sponsor home and the entire property
● Complete required trainings and competencies, as specified by DBHDS, DMAS, HCBS,
and CHCG, as needed
● Train in-house DSPs and ensure that DSPs comply with regulations, to include
documentation, required trainings, routine care and support
● Complete progress notes daily and submit documentation monthly as specified by
CHCG staff
● Attest to the financial capacity to meet the Sponsor’s own expenses for 90 days,
independent of monies receive for residents
● Respect the confidentiality of information concerning the Individual and Common Health
Counseling Group and share this information only with appropriate persons
● Ensure that the Individual is treated with dignity and respect by the Sponsor, agencyapproved DSPs, family members of Sponsors and/or DSPs and other Individuals.

Role as a Supervisor:

Develop and maintain a monthly schedule with all shifts identified and covered by employees who
have attended and completed required training and certifications.

Monitor and evaluate employees to ensure they are performing their duties as a Direct Support Professional.

Complete performance evaluations for employees, probational and annually.

Monitor employees to ensure they are implementing all certifications and training.

Ensure that employees attend orientation and annual certifications and training.

Support employees with providing the Individuals needs for the home and in the community.

Have weekly supervision with employees.

At least quarterly staff meetings to educate staff on annual Individualized Plans
(Service and Behavioral), Assessments (Initial, Comprehensive, and Risk, etc.).

Ensure employees can assess and document on a daily basis how they communicate and understand
the needs, plans, and assessments of the Individuals that are in the home.                                                                                                                                                                                                                                                                                    Minimum Requirements:                                                                                                                                                                                                                                                                                                                                                                              High school diploma or equivalent. A minimum of 6 months of Direct Support Professional
experience is preferred. Completion of minimum of 5 hours of training in approved residential
setting if no prior DSP experience. Valid Driver’s License and DMV driving record. Complete
HOU orientation, as identified in HOU policies and procedures. Complete with a passing score
the following trainings - CPR/First Aid, Medication Administration, Behavioral Management (Crisis
Wave), Human Rights, Infection Control, and all other trainings and certification as identified in
DBHDS regulations.                 
                                                                                                                                                                                                        Desired qualities, skills, and knowledge of a Sponsor:
                                                                                                                                                                                                    Individuals, couples, or families interested in becoming a Sponsor must be willing to open their
homes to individual(s) diagnosed with intellectual and/or developmental disabilities. Ideal
candidates will possess the following qualities:
• A passion and willingness to enhance the lives of others and help others to achieve their
desired goals.
• Superior oral and written communication skills, to include advocacy skills.
• Ability to use a computer and knowledge of person-centered documentation.
• Willingness to learn
• Physical, mental, and emotional stability
• Flexibility, dedication, patience, compassion, organization, initiative, commitment
• At least 6 months of direct care support with individuals with a developmental disability                                                                                                                                                                                                                                                      Licensure:
                                                                                                                                                                                                                     ● Complete a criminal background check/ CPS Central Registry check
 ● Tuberculosis screening
● Valid state driver’s license and current auto insurance
● CPR/First Aid Training Certification and Crisis Wave training (within 15 days of
employment)
● Complete DSP orientation training/test and other required trainings
● Crisis Wave (behavioral management training)
● Medication Management
                                                                                                                                                                                                       Hours:                                                                                                                                                                                             The Sponsor must have schedule flexibility to meet the needs of the Individual(s) 24 hours a
day/7 days a week. The Sponsor must ensure that an CHCG DSP is available to provide
support for the Individual in the absence of the Sponsor.
Compensation Package:
The Sponsor will be compensated monthly as a 1099 contractor. Monthly compensation will be
determined based upon the tier of the Individual(s) and the split determined by CHCG and agreed
upon by the Sponsor and CHCG staff. The agreed upon monthly compensation is stated and
endorsed by the Sponsor and CHCG staff in the Sponsor contract.
