Social Worker

Our spectrum of home-based services now includes Skilled Home Health Care, Palliative Care, Adult Hospice Care and Pediatric Care. We are seeking a Home Health Social Worker that will be responsible for providing medical social casework services for home care patients in accordance with compliance with organization policies and procedures and applicable laws and regulations.

Schedule:

Full-time; Monday-Friday; 8:30am - 5:00pm

Part-time; 3 days per week; 8:30am - 5:00pm

Location:

Sonoma County with occasional coverage to Marin County

By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.

Essential Duties & Responsibilities include (but are not limited to):

• Evaluates the need for medical social casework services to patients in their place of residence

• Participates in ongoing interdisciplinary assessment of the patient

• Assesses social and emotional factors relating to the patient's condition to estimate the capacity of the patient and family to cope with problems of daily living

• Participates in ongoing interdisciplinary assessment of the patient

• Accurately assesses the need for, and performing, revisits

Education/Experience:

• Master’s degree in social work from a school accredited by the Council on Social Work Education or Master’s degree in mental health discipline

• Minimum one-year’s social work experience in a health care setting

Certificates, Licenses, Registrations:

• Current California Driver’s License and automobile insurance that meets State of California requirements, if driving a car.

Pay Range:
$44.50 - $52.30 hourly
