About the Role:
There are 2 Full-Time Ongoing positions at 160 hours per fortnight for two talented Social Workers to join the Complex Care Hub team. The Complex Care Hub is an innovative interdisciplinary service that has been established to lead and coordinate the health care experiences for complex patients and their families. The purpose of this role is to provide social and emotional assessment and intervention for children and families within the Complex Care Hub, where health issues are identified as impacting on quality of life and outcomes for the child. You will use your knowledge of chronic health, disability and community resources to improve the health and wellbeing of children and their families. Some home visits will be required to support families in the community.
Classification for this position with range from SC21-SC24, Grade 2 ,Year 1 to Grade 2, Year 4 (FTE base salary $85,000 to $103,000 per annum, plus superannuation, plus salary packaging).
What you'll achieve:
Provide high quality, evidenced based clinical care to children and families presenting with a broad range of complex medical and psychosocial needs
Engage in quality improvement, research & evaluation projects to enhance existing Social Work, Allied Health services and materials provided to patients and families
Promote and develop partnerships with other health care and community providers / Social Work networks
Your skills and experinece:
Eligibility for membership of AASW and meet AASW CPE accreditation standards
Clinical experience recognised in closely related field of practice
Demonstrated capacity to respond to vulnerable children, trauma and bereavement
Demonstrated knowledge and understanding of disability services and the NDIS
Excellent interpersonal, communication and presentation skills
Ability to work in a multidisciplinary team and effectively articulate the social work role
A strong understanding of community resources and legislation relevant to children and families
Current Full Victorian Drivers Licence
About the Complex Care Hub:
The Royal Children’s Complex Care Hub is an interdisciplinary service that has been established to lead and coordinate the health care experiences for complex patients and their families. Patients with complex needs frequently require multiple teams, advanced technology and medical equipment to support their care needs, both within the hospital and at home. As an integral part of the Royal Children’s Complex Care Hub, the role of the social worker is to deliver and improve service provision for children with complex needs and their families and to initiate strategies to better supports their care and treatment. The social worker will work in partnerships with families of children with complex care needs, and work in close liaison with other service providers both within the hospital and the community.
