Solstice Home Health is seeking an experienced RN Case Manager who operates at a high level clinically, values autonomy, and takes ownership of patient outcomes. The Role

As a Senior RN Case Manager, you will manage a dynamic caseload of home health patients, serving as the clinical leader responsible for care planning, coordination, and outcomes. You will work independently while collaborating with a high-functioning interdisciplinary team.

Key Responsibilities

Perform comprehensive start-of-care, recertification, and routine assessments

Develop and manage clinically sound, outcome-driven care plans

Coordinate care with physicians, therapists, and care teams

Proactively manage patient progress and prevent avoidable hospitalizations

Provide patient and family education with clarity and confidence

Maintain accurate, timely, and compliant documentation

Manage caseload effectively while maintaining high standards of care

Required Qualifications

Active RN license in Utah

Minimum 2+ years of Home Health experience (strongly preferred)

Proven ability to manage a caseload independently

Strong clinical judgment and decision-making skills

Excellent communication with physicians, patients, and families

High level of organization and time management

Reliable transportation and ability to travel locally
