We are seeking a Benefits & Community Resource Specialist to support clients in securing and maximizing the public benefits and community resources necessary for long-term stability and success. This role serves as a key partner in the client journey following intake and clinical assessment.
You will work directly with clients to navigate Social Security (SSI/SSDI), Medicaid, housing assistance programs, insurance enrollment, and representative payee services. This position plays a vital role in helping clients remove financial and systemic barriers so they can focus on treatment, stability, and personal growth.
This is a mission-driven role requiring strong advocacy skills, knowledge of government systems, and the ability to manage detailed documentation and follow-through.
Key Responsibilities
Benefits Navigation & Advocacy
Assist clients in applying for and maintaining SSI, SSDI, and other Social Security benefits
Navigate Medicaid eligibility, renewals, and managed care enrollment
Identify all applicable federal, state, and local public benefit programs
Support clients with SNAP, cash assistance, and other entitlement programs as appropriate
Track application timelines, appeals, and documentation requirements
Housing & Community Stabilization
Assist clients in applying for HUD programs, housing vouchers, and local housing assistance
Coordinate with housing agencies, case managers, and community partners
Support clients transitioning from higher levels of care or unstable housing situations
Advocate on behalf of clients to reduce barriers to housing and community integration
Financial & Payee Support Services
Coordinate representative payee services when appropriate
Assist clients in understanding financial responsibilities and budgeting resources
Provide guidance regarding insurance enrollment, including Medicaid managed care and other coverage options
Help clients obtain additional insurance coverage (e.g., life insurance) when aligned with client goals
Care Coordination & Documentation
Collaborate with clinicians, intake staff, and leadership to identify client needs post-onboarding
Document all services and communications accurately in the Electronic Health Record (EHR)
Maintain compliance with HIPAA and confidentiality standards
Monitor benefit renewals and proactively prevent lapses in coverage

Qualifications
Associate’s or Bachelor’s degree in Social Work, Human Services, Healthcare Administration, or related field preferred (equivalent experience considered)
3+ years of experience in behavioral health case management, benefits coordination, or community resource navigation
Demonstrated experience assisting clients with SSI/SSDI applications and appeals
Strong working knowledge of Medicaid eligibility, renewals, and managed care systems
Experience navigating housing assistance programs (HUD, vouchers, local housing resources)
Familiarity with representative payee services and financial stabilization supports
Strong understanding of HIPAA and confidentiality requirements
Excellent advocacy, communication, and problem-solving skills
Highly organized with the ability to manage multiple cases and complex documentation
Ability to work independently while collaborating effectively with clinical staff
