We are seeking a dedicated and skilled School-based Physical Therapist Assistant to join our team. The ideal candidate will work under the supervision of licensed physical therapists to provide high-quality patient care. Duties:
Assist physical therapists in implementing treatment plans tailored to individual patient needs.
Conduct therapy sessions that may include exercises, modalities, and manual therapy techniques.
Monitor patient progress and report any changes or concerns to the supervising therapist.
Educate patients and their families on home exercise programs and self-care strategies.
Maintain accurate documentation of patient care and progress notes in compliance with regulatory standards.
Collaborate with other healthcare professionals to ensure comprehensive patient care.
Participate in discharge planning to facilitate smooth transitions for patients returning home or moving to other care settings.
