Mountain Community Health Partnership is seeking a full time School-Based Health Programs Coordinator to serve as the primary liaison between the health centers and local schools. This role is essential to the successful planning, coordination, and delivery of school-based medical and dental services that improve access to care for students and families.
The ideal candidate has a clinical background (medical or dental) and strong organizational skills, with a passion for community health, student wellness, and collaborative teamwork.
Key Responsibilities:
Coordinate and manage school-based medical and dental programs, including screenings, preventive services, immunization events, and health education.
Serve as the main point of contact for school administrators, nurses, counselors, and staff.
Manage outreach cycles, schedules, calendars, and on-site workflows in collaboration with school and internal clinical teams.
Ensure all parent/guardian consent forms are properly distributed, collected, verified, and documented.
Coordinate staffing for school events, including providers, assistants, hygienists, medical assistants, and interpreters.
Maintain readiness of portable clinical equipment, supplies, and PPE.
Support on-site clinical operations including check-in, screenings, documentation, referrals, and follow-up care coordination.
Ensure accurate and timely documentation in the electronic health record (EHR).
Monitor infection control, safety and quality standards in school setting .
Track participation, services provided, outcomes, and referrals; prepare reports for leadership and funders.
Support compliance with HIPAA, HRSA/FQHC requirements, and grant obligations.
Provide orientation and workflow training for staff supporting school-based programs.
Support community outreach events related to school-based services.
Identify process improvement opportunities and collaborate across departments to enhance service delivery.
Qualifications:
Education: Clinical background required (medical or dental) such as:
RN or LPN with active state license
Dental Hygienist (RDH)
Dental Assistant
Comparable licensed or certified clinical professional.
Experience: At least one year of experience in clinical operations, community health, school-based care, or mobile/portable health services. Ability to manage multiple priorities in dynamic environments. Frequent standing, walking, and occasional lifting (up to 50Ibs). Bilingual skills preferred.
