About the role:
Reporting to the Nurse Unit Manager, some of your duties for the role will include:
Provide clinical care coordination for people referred to the service.
Developing, implementing and evaluating care plans in conjunction with the person and their family.
Applying evidence based interventions to assist people to manage a complex mental health crisis.
Work in conjunction with the multidisciplinary team to coordinate care, share clinical information, and facilitate seamless transitions between services.
About you:
Registered as a Nurse by the Nursing/Midwifery Board of Australia (NMBA) and who holds a current practicing certificate. Must be enrolled in an approved mental health course
Demonstrated commitment to providing consumer/client and family centred care
Ability to be, creative, innovative and flexible when approaching issues within a healthcare setting
Effective communication skills including, problem solving, conflict resolution and negotiation skills
Registered Nurse/Midwife with at least 3 years, full time equivalent, post registration experience
Experience in the leadership and direction of student nurses, enrolled nurses and less experienced registered nurses
This is a single vacancy, with two possible appointment classifications. The successful applicants will be appointed in the position appropriate to their skills, qualifications, and experience.
Please note that this position is multi-classed as either a AHA1, AHA2 or AHA3 and classification will be determined by qualifications and experience.
