We are growing and currently seeking a Registered Nurse (RN) Case Manager to join our Summit County team. This is an opportunity to build lasting relationships with patients, use your clinical judgment independently, and contribute to a team that puts patient safety and nurse well-being first.

What We’re Looking For

We are seeking a compassionate, dependable RN who:

Holds an active RN license in Ohio with at least 1 year of experience
(Home health or geriatric experience is helpful but not required—we provide training.)

Communicates clearly and professionally with patients, families, and the care team

Is organized, self-directed, and able to manage a flexible schedule

Values teamwork and is committed to delivering high-quality care in the home setting
