Job Description
Country Manor Home Health Care strives to provide superior health care and services by assisting the client to attain his/her maximum potential for wellness and independence. As an RN Case Manager, you can directly help to enrich the quality of life for the individuals and families we serve.
This is a part time, day shift, minimal weekends, and on call rotation.
Flexible scheduling/hours available!
As an RN Case Manager, you will:
Facilitate the clients plan of care
Perform skilled nursing services
Regularly evaluate the client's nursing needs and inform the physician and other personnel of changes in the client's condition
Communicate/Initiate appropriate preventative and rehabilitative nursing procedures
Counsel the client and family in meeting nursing and related needs
Assume responsibility for personal growth and development by participation in in-service programs and other educational programs
Maintain good relationships with clients and their families, staff and community persons
Have a working knowledge of Country Manor Home Health Care policies and procedures
Have a working knowledge of State Licensure Rules and Regulations
Supervise and train LPNs and HHAs in accordance to agency policy
Participate with budget planning and staffing
Collaborate with and acts as a resource to other departments of Country Manor
Perform other duties as assigned
Qualifications:
Must possess a current MN RN license
1+ year of nursing experience preferably in Home Health Care
Broad knowledge of nursing and competence in Home Health Care
Visual and aural acuity to detect changes in clients
Strong written and oral communication skills
Ability to recognize and understand clients needs
Willingness to assume responsibility for caring for and serving the needs of clients in a manner conductive to their safety and comfort
Up-to-date knowledge of current trends and new developments in nursing
Ability to work effectively with others in a cooperative, considerate and helpful manner
Reliable transportation
Willingness to conform to and follow the philosophy and objectives of Country Manor Home Health Care and understand the organizational structure
Patience, consideration and tact when dealing with the resident, family and community contacts
Education:
Must be a graduate of a state-accredited school of nursing
Must be CPR certified.
Must meet educational requirements to maintain certification
Hold and maintain a license granted by the State of Minnesota
