A rapidly growing telehealth organization is seeking a Nurse Care Manager to support patients across multiple states through a fully remote care model.
This role focuses on coordinated, patient-centered care delivery, emphasizing chronic condition management, patient education, and collaboration with interdisciplinary teams to improve outcomes and reduce care gaps.

Key Responsibilities
Deliver comprehensive care management services across diverse patient populations
Conduct patient assessments and develop individualized care plans
Support chronic disease management (e.g., diabetes, hypertension, CHF, COPD)
Provide patient education on conditions, medications, and self-management strategies
Use motivational interviewing techniques to improve adherence and engagement
Coordinate care with providers, pharmacists, and other healthcare professionals
Identify care gaps, risks, and barriers to treatment adherence
Ensure accurate and timely documentation in compliance with program standards
Support care transitions and continuity across settings
Required Qualifications
Active Registered Nurse (RN) Compact License (NLC)
Minimum of 2 years’ experience in care management, case management, or chronic disease management
Willingness to work across multiple state populations
Preferred Qualifications
Experience in:
Chronic care management programs (CCM)
Transitional care management (TCM)
Remote patient monitoring (RPM)
Telehealth or virtual care delivery
Strong background in patient education and engagement
