As a Case Manager, you will play a vital role in supporting patients on their healthcare journey by working closely with providers to deliver high-quality primary care services. This position focuses on chronic disease management, patient education, preventive care, and empowering individuals to take control of their health and wellness.

✨ Position Responsibilities Include:
• Assisting providers in the delivery of primary healthcare services
• Providing health education and chronic disease management support
• Educating patients on diabetes care and preventive health services
• Coordinating care and serving as a case manager for patients with chronic conditions
• Performing designated nursing procedures within the Arkansas State Board of Nursing scope of practice

• Current Arkansas RN or LPN license
• Basic Life Support (BLS) Certification
• Some travel required
