Registered Nurse– Mobile Unit HART Hub *REPOST

New Position

Full-Time Contract until March 31, 2028 (possibility of extension pending funding)

Location: West End Communities – Garden River First Nation, Batchewana First Nation, Thessalon First Nation

Salary Range: $79,748 -$98,080

We work with our 11 First Nation community partners and the IFC in the provision of primary health care, traditional healing, mental wellness and addictions, home and community support services, and health promotion services.

 

The opportunity: The Mobile Outreach Unit Registered Nurse (RN) provides client-centered, trauma-informed, and culturally grounded nursing care within a mobile outreach model serving individuals experiencing mental health challenges, substance use concerns, and related social vulnerabilities. Working across community settings, the RN delivers assessment, brief interventions, harm reduction services, and care coordination while supporting continuity of care between clinical, cultural, and community partners. This role emphasizes relationship-based practice, accessibility, and integrated care for individuals who may face barriers to traditional health systems.

What you need to bring with you:

Registered Nurse in good standing with the College of Nurses of Ontario.

Minimum 2–3 years of clinical experience, with demonstrated experience in mental health and addictions required.

Knowledge of trauma-informed care, harm reduction principles, and recovery-oriented practice.

Experience working with vulnerable or marginalized populations.

Strong clinical assessment, communication, and relationship-building skills.

Valid driver’s license and ability to operate a mobile outreach unit vehicle.

Current CPR and First Aid certification.

Naloxone training (or willingness to obtain).

Assets

Experience in Indigenous health settings and familiarity with culturally grounded approaches to care.

Knowledge of local community resources and social service systems.

Training in crisis intervention, suicide risk assessment, or addictions medicine.

Previous experience in mobile outreach or community-based models of care.

What to expect? Given the traditional practices of Indigenous people, from time-to-time exposure to smoke from the burning of sacred medicines such as tobacco, sweet grass, sage or cedar may occur. Hiring of Indigenous People will be given preference, as allowed under Section 14 of the Ontario Human Rights Code – Special Programs.
