Our team of Account Managers and Staffing Coordinators work together to refer the highest caliber of professionals with degrees, certifications and licenses of varying levels in the fields of social services, psychiatry, occupational therapy, language speech pathology, special education teachers, para professionals, mental health, addictions treatment, allied therapies and pharmacy. We primarily provide service supplemental staffing services to school districts and charter schools, hospitals, community mental health centers, outpatient treatment facilities, and residential homes.
Registered Nurse – Medical Case Manager. Contract to Hire Opportunity
As a Medical Case Manager RN you will receive any children taken into DCFS custody with in 24hrs. You are considered the health works lead agent and will immediately request all medical records and made aware of any conditions of any child in your custody. This will include immunization, dental and vision care. The opportunity is Monday –Friday, first shift, no weekends. This opportunity will work with DCFS caseloads, family and children. This opportunity will also include taking responsibility for maintaining complete documentation for clients and answering any questions foster parents will have. You will also manage a high risk caseload. This is a building based opportunity at our local office in the heart of Peoria.
You will need to demonstrate organization, excellent charting skills and overall a caring disposition and desire to care for our client base. Our client is a Non Profit Organization attached to Human Services; you will provide medical assistance to the community with your cases coming into the building to see you upon your request. Medical Case Manager Responsibilities include:
• Provide case management services to DCFS Children.
• Supporting needs of children and families and plan services.
• Client care documentation required.
• Monitor family progress towards goals in case files.
Qualifications:
• RN; 2 year Degree.
• Experience working with children and families.
• Valid Driver’s license and a car are required.
• Pass background check
