Greenlife Healthcare Staffing is seeking an experienced Registered Nurse MDS Coordinator to join a Skilled Nursing Facility in New York, NY. Key Responsibilities:
• Intake and evaluation process of patients in long-term care facilities.

Requirements:
Qualifications:
• Education: Associate’s or Bachelor’s degree in Nursing (ADN/BSN).
• Licensure/Certifications:
• Active New York RN license.
• MDS Assessor Certification (required).
• Experience: 1–2 years in long-term care with MDS coordination experience.
• Soft Skills: Attention to detail, strong communication, and ability to lead interdisciplinary teams.
