Edison Home Health Care, a leading home health care agency serving all five NYC boroughs, is seeking an experienced Registered Nurse to join our team as a Field Supervisor.
Job Summary: The position is primarily field based, a Registered Nurse (RN) that oversees all aspects of the care being delivered in the home. The nurse provides on-site leadership, guidance and supervision to field employees including nurses and aides and reports back to the Director of Nursing and / or market Clinical Lead. The RN is also the lead field trainer for newly hired RNs and provides ongoing oversight to all RNs to ensure the highest quality in patient care.
Essential Duties and Responsibilities:
Provides field training and routine oversight to new and existing field RNs
Completes (minimally) annual face-to-face in-home supervision of field RN; documents supervisory visit for RN personnel record
Participates and leads regularly in team meetings
Monitors Field Admin RN performance KPIs and addresses deficiencies
Provides RN coaching for performance and performance management as needed under the guidance of the Director of Nursing and / or market Clinical Lead
Participates in monthly Quality and Compliance meetings, supports Quality and Performance Improvement (QAPI) in the market
Provides ongoing feedback to field admin RNs on performance
Assists with managing field response to clinical incidence or challenging client needs
Serves as a resource to field RNs
Conducts patient visits in addition to supervisory responsibilities, fulfilling all aspects of the role of Field Admin RN
What You Bring:
Excellent interpersonal, written, and verbal communication skills
Demonstrated growth mindset with ability to lead others
Intermediate ability to use digital systems such as Microsoft Office, HHAExchange, Sales Force
Education and Experience:
Graduate of an accredited nursing school. BSN preferred.
Minimum of two (2) years clinical nursing experience, preferably in home health/home care setting.
Experience working with medically fragile clients, addressing medical, social and behavioral components of care.
