Since 1970, Salud has been committed to providing a Medical Home to patients, where medical, dental, behavioral health services, and clinical pharmacists work together and coordinate efforts in a team-based system of care. The Clinical Quality Consultant is responsible for integrating Continuous Quality Improvement (CQI) activities throughout each site and consulting with each site in the coordination, implementation, support, and monitoring of these activities.  The Clinical Quality Consultant approaches his/her tasks in a team-based care fashion that supports patients and their families in a self-management, self-efficacy, and behavior change.  
We offer:
• Starting annual salary range:  $68,653 to $78,651
• Monday to Friday – 8 am to 5 pm

Supervision Received:
• Works under the direct supervision of the Quality Director
Supervision Exercised:
• Other Process Improvement members as assigned
Specific Duties:
• Creates and maintains toolkit for Continuous Quality Improvement - CQI 
• Develops training of CQI principles
• Regularly travels to sites to meet with Quality Committees and staff to promote, educate, and train on current Quality Measures and CQI principles
• Attends Quality Steering Committee Meetings to assist in prioritizing and approving CQI measures and activities
• Coaches site Quality Teams in the development of standardized workflows, instructions, and procedures
• Coaches and trains site Quality Team members and Salud Departments to acquire the skills necessary to independently use the CQI process for monitoring, documenting, and reporting on their Quality Measures
• Responsible for tracking and maintaining the Clinical Quality Dashboard
• Analyzes workflows/processes and defines opportunities for clinical quality activities
• Works in conjunction with Medical Providers, Dental Providers, Behavioral Health Providers, and all other staff to ensure that patients obtain the highest standard of integrated healthcare
• Acts as a patient advocate for our vulnerable populations by ensuring that patients are fully informed and comfortable with their visit to Salud
• Follows procedures for the electronic health records system
• Guides site Quality Team activities based on data analysis
• Supports the update of the Quality Plan; aids in the development of the yearly Quality Work Plan
• Maintains Calendar of Activities for each Quality Measure
• Supports the approved quality measures as defined by Salud, which may include activities around Meaningful Use, AAAHC, PCMH, SIM, and other state or federal quality initiatives
 
Education:
• Bachelor’s Degree required
• Level of experience commensurate with a Bachelor’s Degree
 Experience:
• Experience with Process Improvement tools and techniques, required
Licensure/Certification:
• None
Knowledge, Skills, and Abilities:
• Excellent oral and written communication and presentation skills with the ability to effectively adapt content and style to diverse audiences from medical professionals, senior administrative leaders, and other employees, required
• Strong understanding of Continuous Process Improvement methodologies
• Strong analytical and decision-support skills, required
• Strong computer skills including a master of Microsoft Office Suite, especially Excel, Word, and PowerPoint
• Must work well with other patient care team members 
• Strong teaching and coaching skills
• Working knowledge of various software applications within the organization
• Understands and utilizes reporting tools
• Sensitivity to low-income and ethnic minority communities is a must
