QMHP Responsibilities:

As the QMHP, your key responsibilities will involve the following:

Responsible for providing skill-building training in the home or community: one-on-one skills sessions at client's home, at the office, or in the community relating to goals and objectives using evidence-based practices and curriculum.

Provides support and crisis/emergency services for the child/adult/family through face-to-face contact, phone contact, or availability by an on-call system (all time spent on crisis assessment and planning, crisis linkage and follow-up, and stabilization must be documented as crisis hours)

Maintains all client documentation in the electronic record and writes progress notes that adhere to the Texas Administrative Code's requirements.

Utilize practical listening skills to establish rapport with the client, family members, and other individuals close to the client; provide emotional support and encouragement and engage family members in the service process.

Maintains records by regulatory/agency standards and enters data clearly, accurately, and timely. Also, promptly submit documentation needed to track output/outcome measures. May be asked to assist in record review or qualitative review processes.

Responsible for keeping the family and team members informed on progress toward achieving Service Plan goals

Assisted clients with daily living tasks such as behavior management, counseling services, budget management, independent transportation assistance, time management skills, and other tasks to promote independent living.

Collaborates with court/probation, tribal/community agencies, families, and other natural support systems to meet clients' needs and develop strong working relationships with community partners.

Assist clients in practicing effective communication, problem-solving, and anger management techniques daily.

Participate in weekly and monthly staff meetings, company programs, and professional development training as required.
