The Case Manager/QIDP will work at our community day services program where program participants engage in on-site and community-based recreational and educational activities of the program participants' choosing with a few individuals engaging in work activities. The case manager will work with a team to assist individuals with developmental disabilities in achieving their personal goals through advocacy, guidance, and coordination with co-workers and other service entities.

REQUIREMENTS
Bachelor's Degree in Human Services, Psychology, Sociology, Counseling, Social Work, Education, or a related field (Please feel free to contact us if you are unsure whether your degree will qualify.)
Valid/Current Driver's License
One Year of Experience Working with Individuals with Developmental Disabilities (Note: Experience can be personal, professional, intern/practicum, volunteer-related, or a combination of each.)
Must pass a criminal background check, as required by the Healthcare Worker Background Check Act (225 ILCS 46), and a DCFS Child Abuse and Neglect background check
Successfully complete and maintain American Heart Association or American Red Cross CPR/First Aid certification
SUMMARY OF DUTIES
Communicating with families, program participants, and other service providers to coordinate day program services and to ensure that the individuals needs are met.
Responsible for writing service plans and daily/monthly note summaries in addition to related paperwork to facilitate services.
Responsible for the day-to-day supervision and/or coordinating supervision of the day program participants.
Assist in planning/organizing community-based activities for program participants based on their expressed interests.
Must be timely and accurate in completing documentation, including daily attendance for the day program participants.
Must be committed to improving the lives of individuals with developmental disabilities.
SCHEDULE
40 hours per week
Monday through Friday, from 8 am - 4:30 pm (hours may vary slightly, depending on bus/transportation schedules)
Please note that our day programs are closed on weekends and most major holidays.
WAGES
$21 per hour with a $0.50 increase after training is completed
