Psychogeriatric Resource Consultant (PRC)
1 Permanent Part-time – Sarnia Site

Reporting to the Manager of Integrated Client Services, the incumbent will be responsible for the following functions:
Providing geriatric specific assessments and treatment planning.
Liaising, when appropriate, with other care givers/support providers who are involved to ensure continuity of care.
Supporting the implementation of the treatment plan.
Advocates for older adults and minimizes the risk associated with responsive behaviours.
Provides clinical leadership to the Geriatric Mental Health Outreach Team, BSO lead and in house teams as needed.
Collaborates with agencies that provide healthcare, community and social services for older adults.
Identifies potential risks in meeting targets/metrics specific to BSO and collaborates with the Regional Coordinator for solutions and/or advocates for Governance level intervention.
Qualifications:
Bachelor of Science in Nursing degree and Registered with the College of Nurses of Ontario.
Experience and detailed knowledge of serious mental illness, dementia, and other age-related conditions of older adults usually gained by 3 years of experience in working with this population.
Valid Ontario Drivers License and access to a vehicle.
Hours of Work: 28 hours/week. Flexible hours are required to meet the position requirements.
Salary: Commensurate with experience and education within the range: $72,405 - $88,092, inclusive of HOOPP.
