Responsibilities:

Conduct comprehensive assessments of patients’ psychiatric and overall health needs

Develop and implement individualized care plans in collaboration with physicians and the care team

Administer medications and monitor for efficacy and side effects

Provide patient and family education and support regarding psychiatric conditions and treatment plans

Monitor patient progress and document findings in compliance with home health regulations

Communicate effectively with the interdisciplinary team to ensure coordinated, high-quality care

Serve as an advocate for patients, promoting mental health wellness and recovery in a home environment

Qualifications:

Active RN license in Arizona

1–2 years of psychiatric nursing experience preferred

Home Health experience is a plus

Strong clinical skills, excellent communication, and a patient-centered approach
