Psychiatric Mental Health Nurse Practitioner - Outpatient - Pioneer Valley
We are seeking a Psych Nurse Practitioner or CNS to work in a collaborative and innovative team practice that has a commitment to quality and patient centered care in a level III certified medical home.
Provider will Conduct clinical evaluations and develop and implement care plans for patients
Prescribe and Manage Medication and follow up on medication requirements of patients.
Assess physical and psychosocial status of patients by means of interview and by obtaining health and medical history of patient
Evaluate patients based on the patient’s physical health, psychological, social, economic, educational, functional or developmental status as appropriate to the patient’s treatment
Provide consultation to primary care and other medical staff regarding behavioral health issues of individuals and families.
Outpatient only with a flexible Monday-Friday schedule ~Part time or Full time ~ Weekend hours available if desired. Very light call
Extremely competitive salary with sign on bonus, tuition reimbursement, relocation expenses and 4 weeks vacation
