Employed Full time flexible outpatient psychiatric nurse practitioner opportunity on Cape Cod
Practitioner will provide psychiatric mental health assessment, including: description of presenting problem, psychiatric history, family history of mental health problems, relevant medical history, substance abuse history, risk assessment etc.
Provide individualized and culturally sensitive medication management applying evidence-based or best-practice standards where applicable
Develop care plan by analyzing and documenting the patient's physical condition, psychological status, and influencing social factors; applying special training.
Provide clear documentation of all services and interventions
