Cape Cod Health Care Facility seeks a full time Outpatient Psychiatric Nurse Practitioner to serve patients who are homeless, or at-risk of homelessness, many with multiple conditions including mental health and substance abuse issues, in a compassionate, collaborative, educational and caring environment
In this role the NP will provide psychiatric mental health assessment, including description of presenting problem, psychiatric history, family history of mental health problems, relevant medical history, substance abuse history, risk assessment etc.
Provide individualized and culturally sensitive medication management applying evidence-based or best-practice standards where applicable
Provide clear documentation of all services and interventions
