Northern Health are seeking applications from motivated and suitably qualified and experienced Clinicians to undertake the Project Officer role in the newly developing Care of the Older Persons Hub.
Northern Health is the major provider of acute, maternity, sub-acute, mental health, specialist, community and home-based services in Melbourne’s rapidly growing outer north.
Services are provided through our six main campuses:
Northern Hospital Epping, Broadmeadows Hospital, Bundoora Centre, Craigieburn and Mernda Community Hospitals Centres and Kilmore District Hospital. In addition, Mental Health Services are provided at Epping, Broadmeadows, Jacana, Preston, Mill Park, Mernda and Coburg.
About the Role:
Under the Strengthening Medicare Initiative, Northern Health are piloting an integrated outreach model to deliver rapid care and support to older persons in the community. The aim of the Older Persons Care Hub (OPCH) is to provide non-admitted specialist geriatric assessments and rapid short-term interventions to older people in their homes who are at high-risk of hospital presentations.
The OPCH will serve as a central point for enquiries for older adults (over 65 years or over 50 years for Aboriginal and Torres Strait Islanders) living at home, care navigation and service coordination across Northern Health’s catchment.
The Project Officer will be the central coordination point for the delivery of the OPCH pilot. This role is responsible for ensuring the successful and timely implementation of the OPCH, driving key milestones and deliverables required as part of the project. The Project Officer will be responsible for completing reports, collecting and reporting data to the Commonwealth, the Department of Health and Northern Health leadership in line with required timelines.
This position is fixed term full time to 30th June 2026. Note consideration will be given to Part-time applicants.
To be successful in this role you will have:
Minimum 5 years clinical experience in relevant healthcare discipline
Extensive experience in an acute/community healthcare setting – and strong understanding of frail aged care needs
Expert knowledge of the broad health care sector
Experience managing complex service and system-level challenges
Highly developed written and verbal communication skills
Well-developed problem-solving skills, ability to think laterally and design solutions
