Salary Range:  $73,105 - $77,557

Location:  Hybrid - frequent travel within geographical locations within the Maamwesying Ontario Health Team service area

The opportunity: Under the direction of the Maamwesying Ontario Health Team (MOHT) Operations Manager, and in close collaboration with the MOHT Project Management Specialist for Lower Limb Preservation, the Lower Limb Mobile Clinic Project Coordinator is responsible for the development, planning, coordination, and implementation of the Lower Limb Mobile Clinic. This position plays a central role in bringing the mobile clinic from concept to operational reality, ensuring that the initiative improves access to culturally safe, community-based services for First Nations community members who live with or are at risk of diabetes and vascular disease.

The Lower Limb Mobile Clinic aims to prevent complications, including non-traumatic lower limb amputations, by delivering on-site assessments, diabetic foot care, wound care, vascular and lower limb assessments, education, navigation support, traditional healing or medicine, and referral services in a fully equipped, culturally safe mobile clinical environment.

What you need to bring with you: 

Diploma or Bachelor’s degree in a health-related field is strongly recommended

Project Management training or certification required or considered to be a strong asset

Wound care, diabetes management, or vascular disease knowledge is considered an asset

Minimum of 2 years of experience in project coordination, preferably in health care or community health

Experience working with First Nation communities

Experience in community-based program development, mobile/outreach health services, or clinical program planning is an asset

Experience with policy and procedure development is an asset

Excellent ability to see the big picture, anticipate problems, organize and coordinate appropriate response

Demonstrated knowledge of current trends in general health care and community resources

Ability to adapt and problem‑solve in dynamic, non‑traditional clinical environments

Ability to work independently while traveling between communities and managing time in remote or rural settings

Ability to meet the physical and sensory demands of the job

Valid driver’s license with access to reliable vehicle

Ability to frequently travel across Northeastern Ontario

A clear criminal reference check is required
