Job Description
As a Population Health Lead, you will serve as a primary driver of the Healthy Planet ecosystem. This is a high-visibility role designed for a seasoned professional who thrives at the intersection of clinical data, value-based care, and technical execution.
You will take full ownership of diverse population health initiatives, transforming complex data into actionable insights while serving as a confident representative of the team to executive leadership and key stakeholders.
Location: 25% on-site in Palo Alto (approximately once per month)
Pay: $132k - $175k salary based on experience
Key Responsibilities
In this role, your day-to-day will involve both high-level strategy and hands-on technical configuration:
• Advanced Analytics & Reporting: Lead the design and build of new clinical dashboards. You will develop specialty-specific reporting frameworks using Reporting Workbench and SlicerDicer to improve patient outcomes.
• Value-Based Care Support: Manage technical initiatives surrounding innovative payment models, including surgical bundles and shared-savings programs.
• Strategic Collaboration: Act as a liaison between the organization and external entities such as CMS and LEAP, ensuring our population health goals align with regulatory and industry standards.
• Registry Management: Maintain and optimize registries and quality metrics to track the health of specific patient populations accurately.
• Stakeholder Engagement: Present project updates, data findings, and strategic recommendations to executive leadership with clarity and professionalism.
Requirements:
Required Experience & Skills:
• 6–10 years of professional experience in healthcare IT or clinical informatics.
• Direct experience in Population Health management and value-based care workflows.
• Certification: Epic Healthy Planet certification is strongly preferred; Epic Ambulatory certification is required.
• Communication: Exceptional verbal and written communication skills with a proven "executive presence."
• Resilience: Ability to maintain a high level of professionalism and composure in fast-paced, high-pressure environments.
Technical Competencies:
• Proficiency in Epic’s data visualization tools (SlicerDicer, Dashboards).
• Deep understanding of clinical registries and quality metric logic.
• Familiarity with cross-functional team dynamics in a large-scale health system.
