The Physical Therapist is responsible for developing a physical plan of care based on a complete assessment and the direction of the patient's physician, integrating the physical therapy into the total Home Health plan of care and coordinating services with other team members involved in the plan of care.  The Therapist may be requested to evaluate Hospice patients for palliation of symptoms that restrict mobility, transfers and ADLs.
What is required...
• Graduation from a certified Physical Therapy program & currently licensed in the State of Washington
• Current CPR card (maintenance required)
• Reliable transportation and automobile liability insurance
• Valid WA State Driver’s License
What is preferred...
• LSVT BIG certification
• Previous Home Care or in/outpatient experience
Wage:  Wage range currently starts at $51.00/hour and goes up to $77.30/hour. Actual wage will be based on years' of applicable experience.
FTE:   1.0 (40 hours/week)
