Position Summary
We are seeking a compassionate and motivated Physical Therapist to join our team in providing specialized home care services to individuals with Intellectual and Developmental Disabilities (IDD). This role offers a unique opportunity to work one-on-one and in small groups within structured group home settings, helping individuals improve mobility, strength, balance, and overall functional independence.
As a family-owned and operated accredited home health agency, we focus exclusively on serving individuals with intellectual and other developmental disabilities in a controlled environment designed to promote dignity, safety, and respect. Unlike traditional home care roles that require extensive travel across multiple counties, our therapists enjoy limited driving—typically no more than 15 miles between locations.
Service Area
Currently serving Stark, Summit, Medina, Cuyahoga, and Lorain counties, with continued growth and expansion.
Key Responsibilities
Perform comprehensive physical therapy evaluations for individuals with IDD.
Develop and implement individualized treatment plans to improve mobility, strength, endurance, balance, coordination, and functional independence.
Provide physical therapy services in group home settings on an individual and small-group basis.
Address mobility challenges, gait abnormalities, transfer training, fall prevention, and adaptive equipment needs.
Collaborate with nurses, therapists, caregivers, and group home staff to ensure coordinated, person-centered care.
Educate caregivers and staff on therapeutic exercises, positioning, mobility techniques, and safety practices.
Monitor patient progress and adjust treatment plans as needed.
Complete accurate and timely documentation in accordance with agency policies and regulatory standards.
Qualifications
Current state licensure as a Physical Therapist.
Home health and/or experience working with individuals with Intellectual and Developmental Disabilities preferred.
Strong clinical reasoning and interpersonal skills.
Ability to work independently while functioning effectively as part of an interdisciplinary team.
Valid driver’s license and reliable transportation.
