Job Description

La Vita Bella Home Health is seeking a compassionate and skilled Physical Therapist to join our growing home health team. We are committed to delivering high-quality, patient-centered care that promotes independence, safety, and improved quality of life in the comfort of our patients’ homes.

Position Type

Part-Time / PRN (Flexible Scheduling Available)

Responsibilities

Perform comprehensive physical therapy evaluations in the home setting

Develop, implement, and modify individualized treatment plans based on physician orders

Provide therapeutic exercises, gait training, balance training, and functional mobility interventions

Educate patients and caregivers on home exercise programs, safety, and fall prevention

Accurately document evaluations, visits, and progress in compliance with CMS and agency standards

Collaborate with physicians, nurses, and the interdisciplinary care team

Maintain compliance with all federal, state, and accreditation requirements

Qualifications

Current Physical Therapist license in the state of practice

Graduate of an accredited Physical Therapy program

Home health experience preferred (not required)

Strong clinical judgment, time management, and communication skills

Ability to work independently in a home-based environment

Reliable transportation and valid driver’s license
