We are seeking a motivated and experienced PHC Field Supervisor to join our team and help ensure excellence in service delivery, compliance, and staff performance.
Position Summary
The PHC Field Supervisor oversees and coordinates the delivery of Personal Assistance Services (PAS) to clients in accordance with state regulations, agency policies, and care plans. This role involves conducting supervisory visits, training attendants, maintaining communication with clients and caregivers, and ensuring compliance with HHSC and agency standards.
Key Responsibilities
Conduct initial, supervisory, and follow-up home visits with PHC clients to ensure quality and satisfaction of care.
Monitor attendant performance, & documentation
Provide training, coaching, and performance feedback to attendants to promote best practices in client care.
Coordinate with office staff to ensure accurate service delivery and timely reporting.
Assist with incident reporting, plan of care updates, and annual reassessments.
Ensure all field activities meet HHSC and agency compliance standards.
Maintain accurate documentation and submit reports promptly.
Support recruitment and retention efforts for qualified attendants.
Serve as a liaison between the office and field staff, promoting communication and teamwork.
Qualifications
High school diploma or GED required
Minimum 1 year of experience in home health, PHC/PAS, or related healthcare supervision role.
Strong understanding of Texas PHC/PAS program requirements and HHSC compliance.
Excellent communication, organization, and leadership skills.
Must have a valid driver’s license, reliable transportation, and proof of insurance.
Proficiency in Microsoft Office, EVV systems, and EMR documentation preferred.
