Job Description

Under the direction of the Manager of Housing, Facilities & Employment Services, the Safe Beds Program Supervisor is responsible for supervising the daily operations and service provision coordination of the Safe Beds program. They are also responsible for providing clinical and crisis leadership to the front-line service delivery staff.

The Safe Beds Program Supervisor supervises the ongoing provision of specialized, comprehensive community-based treatment, rehabilitation and support to Police and Justice System referred Windsor-Essex County area individuals who are experiencing a mental health and/or addictions crisis, who are homeless, and who are involved in, or at risk of, becoming involved in the criminal justice system. The position accomplishes this in consultation with the Program Manager through program development, referral liaisons and referral pathways, identifying and measuring outcomes, community resource partnership building and continuous quality improvement initiatives.

In addition to program coordinating activities and operations leadership to the clinical team, the position oversees front line professional case management services within the framework of a client directed clinical/rehabilitation case management model.  This model includes a continuum of functions, including: assertive outreach, building therapeutic relationships, developing care plans with clients, client and systems advocacy, symptom management, life skills teaching, trauma-focused supportive counseling, motivational interviewing, family support, and crisis intervention, including wellness checks, crisis counselling, and risk mitigation plans. Services are delivered to individuals and their families (or their personal support networks). 

 

RESPONSIBILITIES & DUTIES:

Responsible for the day-to-day operations of Safe Beds, including, immediate risk assessment and mitigation, SOR reporting, task delegation, primary consultant with the Program Manager, team meeting coordination and facilitation and follow up, providing program updates to the Safe Beds team, and coordinating Facility Services and Information Technology Services supports to address Program needs. Responsible for day-to-day implementation and management of the clinical service delivery system for the client service population referred to Safe Beds

 

Provides clinical leadership, education and coaching to the front-line Safe Beds team. This includes, but is not limited to training, orientation and support to new staff members, volunteers, and students assigned to the program and substitutes for and assists other agency staff where skills are applicable. Provides ongoing support and assists in evaluation and skill development of staff, students and volunteers.

 

Manages direct reports including but not limited to: scheduling, delegation of work, hiring, day to day supervision, managing performance, coaching, conducting performance appraisals, etc.

 

On-site, day to day supervision of the Safe Beds staff, including providing mental health education/training, direction, support, and other relevant educational resources to the team as it relates to Safe Beds and the program’s service mission.

 

Developing and fostering relationships with various local police services and associated integrated mobile mental health response teams. by facilitating communication and providing ongoing information/education and program updates regarding Safe Beds programming.

 

Developing, evaluating, improving and expanding Safe Beds programming while participating in creating critical safety protocols. Works collaboratively with the Program Manager on program evaluation and change management, developing recommendations and strategies for overall program improvement.

 

Identifies client needs through ongoing assessment and treatment/care planning, facilitates case conferences where appropriate, and acts as a clinical lead and working collaboratively with Residential Support Workers, Addiction Support Workers, and Peer Support Workers to facilitate implementation of all aspects of the care provision outlined in the Independent Living Survey and associated recovery development tools.

 

Client orientation. Ensuring clients understand the services being offered as well as an understanding of processes, rights and expectations for participation in the program as well as limits to confidentiality.

 

Evaluates clients upon entry to services and throughout service delivery for levels of risk including risk of harm to self and harm to others. Develops risk mitigation plans and strategies to ensure staff are able to effectively work with each client based on the presenting risk factors.

 

Provides psychoeducation about illness, symptom management, medication side effects, and coping skills. Education includes methods for dealing with conflict, anger management, reactivity, impulse control, and other triggering behaviours.

 

Developing and evaluating group educational programs relevant to clients as directed by the manager.

 

Working collaboratively with clients, families, various local police services and associated integrated mobile mental health response teams , probation/parole officers, courts, and other community and justice partners, to implement treatment/care plans that focus on strengths, wellness and empowerment to support recovery and goal achievement. This may include case conferences or other staff consultations.

 

Provides education to clients and families in assisting with re-integration into the community during and following involvement in the judicial system. Community reintegration may include return to school, work and social activities as well as the building of a healthy informal support system.

 

Client and Systems advocacy and linkage to resources: assists clients to access information as well as internal/external resources, services or supports which are deemed necessary. Where resources do not exist or they are inadequate, advocates within system to develop or improve essential services and resources for clients.

 

provides life skill coaching to support activities of daily living and independence. This includes assisting clients in accessing basic needs such as, financial assistance and benefits, emergency and/or stable safe affordable housing, food banks, etc.

 

Provides weekly on-call clinical and operational support, on a rotational basis, for the Agency between the hours of 1630 and 0830 on weekdays and 24 hours/day on weekends and statutory holidays.

 

May be required to provide in person, on site staff coverage in extenuating circumstances between the hours of 1800 and 1000 on weekdays and 24 hours/day on weekends and statutory holidays.

EDUCATION & SKILLS:

Requires the knowledge and skill acquired through the completion of a four-year university degree in a discipline deemed relevant by the employer (i.e. BSW, BScN) from an accredited university combined with significant case management experience. Registered member in good standing of a professional college deemed relevant by the employer such as the Ontario College of Social Workers and Social Service Workers (OCSWSSW), the College of Nurses of Ontario or the College of Psychotherapy in good standing is required.

Three (3) years recent, relevant case management and outreach experience working with vulnerable populations who are experiencing concerns including but not limited to: homelessness, addictions and mental illness. Experience in the justice system and applicable mental health legislation.

Demonstrated leadership and/or supervisory experience. Completion of leadership courses is an asset. Experience in a unionized environment and bilingual in French will be considered assets.

Comprehensive knowledge of mental illness and treatments; comprehensive knowledge of functions and principles of case management; knowledge of relevant mental health policy, legislation and regulations; knowledge of community resources; knowledge of the criminal justice system and court processes; knowledge of homelessness, housing, addictions and treatment, proficiency required with MS Windows Office Suite programs, i.e., Word, Excel, Outlook, and PowerPoint; proficiency in French language would be considered an asset.

 

Hours of Work:       Thirty-Five (35) hour work week. The hours of operation are Monday to Friday, 10:00 AM to 6:00PM. Flexible hours are required to meet client and service needs.

 

Salary range:           $66,767.00. – $81,885.00 annually.

                                 This salary range represents Steps 1 through Job Rate for Band 4 and is inclusive to the maximum earning potential of this role. Compensation within the range will be based on the experience of the incumbent.

 

Compensation includes a competitive and comprehensive benefit, Healthcare of Ontario Pension Plan (HOOPP), vacation and sick leave package.
