Job Description

The Director oversees and optimizes primary care services, ensuring high quality patient care, efficient operations and adherence to clinical standards. Under the direction of the CEO, the Director oversees the clinical management of designated CHC and Interprofessional team.  This role oversees all the client and community facing programs and services at CMHA’s Community Health Centre including our primary health care, chronic disease and mental health, preventative care and population health and wellness portfolios. The Director is responsible for the overall program and clinical oversight to assure consistency, integration and continuity in the provision of services across CMHA and community agencies in alignment with Board, Ontario Health, and provincial strategic priorities. The Director collaborates with internal and external partners to foster innovation for the development and implementation of primary care core services, care pathways and will oversee community focused projects related to Locally Driven Population Health Models.

 

The incumbent will be responsible for budget planning, monitoring and reporting processes, ensuring completion of forms/statistics, participating in analysis, and submitting reports to the Ontario Health/Ministry of Health as required.  The Director shall ensure that CHC sector mandates are reviewed, and information is captured effectively through the EMR system.  The Director shall develop and lead the Health Centre Quality Improvement program and present to the CMHA Board as needed.

 

RESPONSIBILITIES & DUTIES:

The position is responsible for the following:

Supports the vision and mission of the organization.

Fosters a culture of excellence, innovation and quality in primary health care and clinical therapy.

Provide clinical leadership and supervision to all clinical staff consistent with management best practices.

Actively participate in staff, team and interdisciplinary committee meetings.

Establishing and maintaining networks and partnerships with agencies and community groups as needed.

Promoting awareness of the CMHA Health Centre and therapy services and programs.

Create, review and implement medical directives as needed.

Develop and implement infection control practices appropriate for primary health care.

Develop and monitor patient feedback mechanisms and participate in Quality Improvement activities.

Manage program staff by: recruiting staff in compliance with legislative requirements, Collective Agreements and Health Centre policies, orientating and training staff to ensure they have knowledge and insight into the needs of the priority population(s), develop goals and objectives with staff that meet the agency’s strategic plan.

Meet with staff on a regular basis to provide supervision, feedback and monitor program delivery, ensure formal performance appraisals and annual learning plans are conducted on a regular basis and provide professional learning activities for staff to maintain and enhance their skills.

Prepare funding proposals as required, manage and monitor program budgets and complete required reports to funders.

As a member of the senior leadership team, contributes to the development of the Health Centre’s budget.

Ensure the collection of data, completion of reports and recommendations related to primary health care planning as required by funders, other Community Health Centers (as required) and the CEO.

Direct the Health Centre’s activities in primary health care to collect, analyze and report on data and relevant information, and participate in research.

Provide support for organizational change management activities.

Provide leadership to internal Branch committees within area of responsibility and to other committees as required and assigned.

Serve as a staff resource to Branch committees as well as external committees or task forces as assigned.

Other duties as assigned and which are within the scope of the position and which further the objectives/goals of the organization.

 

Leadership and Strategic Direction

Provide visionary leadership and direction for the development, implementation, and evaluation of integrated primary care services at CMHA WECB.

Drive the strategic alignment of primary care services with the organization's mission, vision, and strategic priorities.

Foster partnerships with community physicians, primary care teams, hospitals, Ontario Health Teams (OHTs), and other stakeholders to promote integrated, client-centered care.

Lead quality improvement and population health initiatives that support Ontario Health’s Quintuple Aim, with a particular focus on access, equity, and experience of care.

Identify service gaps, emerging health needs, and innovative approaches to improve client outcomes and enhance team performance.

Operational Oversight and Clinical Integration

Oversee the day-to-day operations and performance of primary care services, including collaborative care models, interdisciplinary team functioning, and service workflows.

Provide leadership and mentorship to a multidisciplinary team including nurse practitioners, physicians, RNs/RPNs, social workers, and allied health professionals.

Ensure compliance with all relevant legislation, professional standards, Ministry of Health guidelines, and CMHA policies.

Ensure the integration of physical health and mental health services, with a focus on improving chronic disease management, preventative care, and holistic wellness.

Maintain current knowledge of clinical best practices, regulatory updates, and innovations in primary care and community-based models.

Data Oversight and Performance Monitoring

Provide strategic leadership for the collection, analysis, and application of health and service delivery data to inform decision-making and continuous improvement.

Lead the development of clinical dashboards, performance scorecards, and quality indicators aligned with program goals and system expectations.

Ensure accurate and timely submission of data and reports to funders and system partners.

Collaborate with the Performance, Quality, and Evaluation teams to track outcomes, inform program planning, and support evaluation of clinical interventions.

Utilize data to monitor client access, equity, health outcomes, and satisfaction to guide service improvements.

Program and Policy Development

Lead the development and ongoing refinement of primary care clinical protocols, referral pathways, and service delivery models.

Participate in the creation and review of internal policies and procedures that affect primary care operations and client experience.

Contribute to the development of CMHA’s operational and strategic plans, ensuring primary care perspectives are embedded.

Work with the Senior Leadership Team to lead and participate in accreditation and quality improvement initiatives.

Partnerships and System Collaboration

Serve as CMHA’s lead representative in primary care partnerships, OHT planning tables, and cross-sector collaboration efforts.

Cultivate and maintain relationships with external partners including hospitals, family health teams, community organizations, and system planners.

Support planning for the integration of new primary care services and contracts, including advocacy for resources and sustainability.

Departmental and People Management

Supervise, mentor, and develop a high-performing primary care team through coaching, professional development, and performance management.

Promote a positive, collaborative, and accountable team culture that reflects CMHA’s values.

Oversee workforce planning, staffing, and succession planning for the primary care unit.

Manage departmental goals and projects, aligning them with organizational strategic directions and tracking measurable results.

Governance and Executive Support

Serve as a staff resource to the CEO, Board of Directors, and relevant Board Committees (e.g., Quality, Strategic Planning), providing performance and service updates.

Prepare reports, presentations, and recommendations that support the Board’s oversight of clinical performance and service alignment.

Advise the CEO and Senior Leadership Team on primary care trends, risks, and opportunities, contributing to strategic and operational planning.

Risk Management and Compliance

Ensure risk management strategies are in place for clinical care, privacy, infection control, and emergency response in primary care settings.

Implement systems and protocols to ensure high standards of care, ethical practice, and safety.

Support the development of mitigation plans and remedial actions when risks or quality issues are identified.

Provides weekly on-call clinical/operational support, on a rotational basis, for the Agency between the hours of 1630 and 0830 on weekdays and 24 hours/day on weekends and statutory holidays.

Member of the Agency Code Response team, providing emergency support and leadership to staff and visitors during and after Emergency Code Events

EDUCATION & SKILLS:

A Master’s degree in Social Work, Nursing, Social/Health Science or an equivalent combination of recent and relevant education, BScN, BA, BSW and related experience. Certificate in Health Services Administration would be an asset. Registered member in good standing of a professional college (Registered Social Worker or Registered Nurse or others deemed relevant by the employer) is required.

Seven (7) years program management experience with progressive responsibility in a community health or social services setting.

Knowledge of serious mental illness gained by experience working with this priority population. Experience in program development, implementation, monitoring and evaluation. Demonstrated ability to work effectively in a multi-disciplinary team environment. Demonstrated ability to supervise, motivate and direct staff to ensure an effective and efficient working environment.  Demonstrated commitment to and knowledge of community-based health care and resources.

 

Hours of Work:       Thirty-Five (35) hour work week. The agency’s regular hours of operation are Monday to Friday, 8:30 AM to 4:30 PM. Flexible hours are required to meet client and service needs.

 

Salary range:           $116,855.00 – $143,544.00 annually.

                                  Compensation within the range will be based on the experience of the incumbent.

 

Compensation includes a competitive and comprehensive benefit, Healthcare of Ontario Pension Plan (HOOPP), vacation and sick leave package.
