Personal Support Worker (PSW) – Rehab – Long Term Care

If you were working for us as an Personal Support Worker – Rehab, here are some of the things you would have done last week:

• Supported Residents with rehabilitative and restorative care activities, including mobility, ambulation, transfers, ADLs, dining support, and active range of motion
• Worked closely with the Rehab Lead and interdisciplinary team to implement, sustain, and improve the Home’s Rehab Program
• Assisted with documentation and evaluation of rehab outcomes, ensuring information was accurate and up to date
• Mentored and supported on-floor staff in Nursing Rehab practices and principles
• Participated in education, in-services, and coaching related to rehabilitation programs and safe mobility
• Used problem-solving skills to support complex rehab situations while maintaining Resident safety and dignity
• Communicated effectively with Residents and families regarding rehab approaches and progress
• Applied principles of Continuous Quality Improvement to daily practice
• Promoted Resident-centred care while supporting both team and organizational goals
• And assisted the team as required

To be hired into this role, you must have:

• Personal Support Worker (PSW) or Health Care Aide (HCA) Certification
• Restorative / Rehabilitation Certification
• Physically able to transfer/transport Residents, tables, chairs, and equipment
• Vulnerable Sector Screening (VSS) check completed within the past 6 months
• TB 2 Step updated within the last year
