Per Diem Registered Nurse (Denver, CO / Field-based)

Overview

• Development of Care Plans supporting member authorizations and renewals

• Support supervisory visits for all members on care supporting onsite requirements for the Denver market

Responsibilities

• Under the direction of the Lead Registered Nurse, support home care consumers in development of Care Plans for start of care and continued authorizations

• Outreach to consumers to ensure development of Care Plan in a timely manner

• Engagement and positive relationships with Case Mangers and Providers to ensure continuity of care for home care members

• Conducting home visits for consumers in designated region to support supervisory visits and ensuring compliance

• Document all interactions consistent with policies and procedures and ensuring HIPAA regulations are adhered to

• Escalate critical incidents by reporting to appropriate contact

• Cross collaboration with pre-care, care and clinical team to ensure appropriate care provided

Education and Experience

• Valid and active RN License in state of Colorado  

• 2+ years clinical experience; Home Care experience preferred

• Proficient in use of Microsoft Office 

Qualifications

• Ability to provide support to our patients in Denver and Northern Colorado; onsite, telephonic and/or virtual  

• Experience working in startups

At FreedomCare, base pay is one part of our total compensation package and is determined within a range. This provides our employees with the opportunity to professionally grow and develop within a role. The base pay range for this per diem role is between $40.00 and $42.00 per hour at the commencement of your first year of employment. Compensation decisions are dependent upon a variety of factors which may include, but are not limited to: skill set, years of relevant experience, education, location, and licensure/certifications.
