Key Responsibilities
• Coordinate and manage home care cases and caregiver scheduling
• Serve as the primary point of contact for patients, families, and caregivers
• Handle high-volume calls, follow-ups, and patient inquiries
• Ensure accurate documentation and compliance in HHA Exchange
• Collaborate with nurses, case managers, and clinical teams to support Patients needs
• Monitor attendance, coverage, and missed visits
• Maintain daily, weekly, and real-time updates on case status
• Provide excellent customer service in a fast-paced healthcare environment
• Any other duties assigned by the manager.
Requirements:
• 1+ year experience in home care, healthcare coordination, patient services, or medical office setting preferred
• Strong knowledge of home care, scheduling, or case coordination
• Experience with HHA Exchange, EVV systems, or healthcare EMR platforms is a plus
• Excellent communication, multitasking, and problem-solving skills
• Ability to manage high-volume workload and prioritize effectively
• Bilingual (Spanish/English) strongly preferred
