Position Summary
The Patient Care Coordinator supports rehabilitation services across our Skilled Nursing Facility (SNF/LTC), outpatient, and home health programs. This role ensures smooth patient intake, insurance authorization, scheduling coordination, and seamless transitions between care settings.
The ideal candidate is highly organized, knowledgeable in rehab services and Medicare guidelines, and committed to delivering excellent patient-centered service.
Key Responsibilities
Coordinate therapy referrals, admissions, and start of care across SNF/LTC, outpatient, and home health settings
Manage therapy scheduling and caseload coordination
Track authorization limits and prevent service interruptions
Support care transitions (hospital to SNF, SNF to outpatient/home health, etc.)
Collaborate with interdisciplinary teams, including nursing, social services, and therapy staff
Oversight of auditing and tracking systems to ensure compliance with regulatory and department policies.
Qualifications
2+ years' experience in SNF, outpatient rehab, or home health preferred
Basic knowledge of Medicare B and insurance authorization processes
Strong organizational and multitasking skills
Excellent communication and customer service abilities
Experience with EMR systems and Microsoft Office
